  1. Number of X-Rays performed
  2. Number of CTs performed
  3. Number of USGs performed
  4. Total number of investigations done
  5. Percentage of reporting errors
  6. Percentage of Number of re-dos
  7. Percentage of reports correlating with clinical diagnosis
  8. Percentage of employees adhering to safety precautions
  9. Total number of employees sampled (denominator)
  10. Waiting time for the investigation
  11. Time for the procedure (summation of TATs for each procedure)
  12. Time for reporting (summation of the TATs for reporting)
  13. Incidence of falls
  14. Percentage of Near misses
  15. Incidence of Parenteral exposure
  16. Incidence of blood & body fluid exposure
  17. Number of Sentinel events reported, collected and analysed within the defined time frame
