List of Quality Indicator for ICU required for NABH accreditation

1. Time for completion of initial assessment. 
2. Sum of time taken for Discharge.
3. Total number of patients (24 hours patient census).
4. Percentage of in-patient case records wherein the care plan with desired outcomes has been documented.
5. Percentage of patients files with documented nursing care plan.
6. Percentage of inpatient case records where in the nutritional assessment has been documented.
7. Number of medication errors.
8. Total number of patient (bed) days.
9. Percentage of patients admitted with adverse drug reactions.
10. Total number of discharges/death/LAMA/DAMA/DOR.
11. Percentage of patients receiving high risk medications who have an adverse drug event.
12. Percentage of transfusion reactions.
13. No. of blood and blood components units wasted after issued.
14. Number of patients with urinary catheter who developed UTI.
15. Number of patients on ventilator who developed VAP.
